Housing Case Manager
Position Summary:
The Housing Case Manager provides individualized support to individuals and families seeking housing stability. This role coordinates services, manages referrals, and ensures compliance with HUD and local housing standards.
Responsibilities:
Develop individualized housing plans and conduct needs assessments.
Coordinate with landlords, housing agencies, and community organizations.
Provide supportive counseling and track resident progress.
Maintain complete and compliant case documentation.
Qualifications:
Bachelor’s in Social Work, Human Services, or related field.
2–4 years in housing support or case management.
Knowledge of HUD guidelines and local housing networks.
